BrightPath is opening a new short-term crisis stabilization home in the Twin Cities metro this September, and we're hiring the Program Coordinator who will help run it from day one.

This is a hands-on leadership role. You are not behind a desk — you work five direct care shifts a week, in ratio, alongside the team you're training. You'll know every young person in the home because you're in it with them.

Because the program is new, you get something rare: a chance to build the routines, the medication systems, and the documentation habits correctly the first time, instead of inheriting someone else's shortcuts.

About the Program

• Two-bed, 245D-licensed residential crisis stabilization home serving youth on DD or CADI waivers
• Short-term by design — the county length-of-stay target is 90 days, and transition planning starts at intake
• Referrals come through the Metro Crisis Coordination Program (MCCP) for the seven-county metro region
• Acuity-based staffing from 1:1 to 3:1, with 24/7 awake coverage
• Located in Anoka or Ramsey County (site being finalized)

Who You'll Serve

We would rather tell you now than have you find out in week three. The young people referred here are in real crisis — losing a placement, boarding in a hospital, or at risk of commitment. Presentations commonly include physical and verbal aggression, self-harm and suicide risk, elopement, and property destruction. Many carry trauma histories and co-occurring diagnoses.

They are also kids who need meals cooked, rides to school, someone to notice when a medication isn't working, and an adult who stays calm. That is the job.

How This Differs From a Standard Program Coordinator Role

In a long-term home, you build a rhythm with the same people over years. Here, you rebuild it every few months. That means:

• Intake moves fast — you'll set up medication systems, documentation, and daily structure for a new arrival on short notice
• Baseline behavioral data matters enormously, because the behavior support plan gets updated every two weeks based on what you and your team record
• Discharge is the goal, not a disruption — you'll help prepare the receiving provider so what worked here keeps working there
• Acuity is higher and staffing is denser, so your coaching of newer staff happens in real time, on shift

What You'll Do

Direct care and program implementation

• Provide direct care and support based on each person's Support Plan and Support Plan Addendum
• Support daily cares, ADLs, medical needs, and mental health supports
• Implement person-centered programming in alignment with the Support Plan Addendum and Individual Abuse Prevention Plan
• Assist in developing goals, monitoring progress, and collecting program data
• Complete and distribute progress reports
• Participate in interdisciplinary team (IDT) meetings and contribute to care planning and transition coordination
• Handle meal planning, grocery shopping, and household supplies, ensuring three nutritious meals daily per licensing standards
• Oversee site cleanliness and make sure assigned cleaning tasks are completed each shift

Health needs and medication management

• Oversee healthcare coordination, including medication orders, inventory, and Medication Administration Review with the Registered Nurse
• Track medical appointments, update records in Therap E-MAR via the Pharmacy Message interface, and document after-visit summaries accurately
• Review Medication Administration Records regularly, resolving gaps and missing entries
• Coordinate medication reconciliation at intake and prepare medication documentation for discharge
• Escalate discrepancies to the nurse and Program Manager promptly

Team support and training

• Train new staff and coach the team on care plans, ADLs, and active treatment plans
• Maintain house and client binders to agency and licensing standards
• Work with centralized scheduling to ensure adequate coverage
• Help facilitate regular staff meetings

Compliance and documentation

• Ensure compliance with licensing standards, including quarterly fire and weather drills
• Document program data and daily logs accurately and on time
• Review data to confirm each shift has the correct number of entries for the program and staffing ratio
• Follow up with staff on behavioral incidents and safety concerns, ensuring proper documentation and reporting
• Complete quality improvement tools and attendance records

House budget

• Allocate Divvy transactions correctly and maintain accurate records
• Upload receipts, verify purchases, and follow budgeting guidelines and authorized-use policies

Schedule

Full-time, non-exempt. Five in-ratio direct care shifts per week, including a weekend rotation and participation in the on-call rotation. Openness to morning, evening, or overnight shifts as program needs require.

Qualifications

• A minimum of 50 hours of education and training related to human services and disabilities
• Experience supporting a vulnerable population in a healthcare oversight capacity
• Leadership experience and/or training in disability services
• Ability to collaborate with a team on healthcare needs, care plan management, and IDT coordination
• Proficiency with electronic documentation; experience with Therap, Google Workspace, or similar systems
• Must pass a Department of Human Services background study and maintain qualified status
• Current valid driver's license and insurance as required by state law

Preferred

• Experience supporting minors or adolescents in a residential setting
• Experience in crisis, behavioral health, or high-acuity residential programs
• Familiarity with 245D standards, the Positive Support Rule, and behavior support plan implementation
• Experience with Therap E-MAR and pharmacy interfaces
• Experience opening or standing up a new program

What's Behind You

• Program Manager on site who owns intake decisions, behavior support planning, and crisis response direction
• Clinical Director for case consultation and incident debriefing
• Internal nursing for medication oversight and individualized health training plans
• In-house CPI-certified instructors — staff receive an 11-hour CPI course plus trauma-informed and DBT-informed skills training, well beyond 245D minimums
• Centralized recruiting and scheduling — we intentionally over-hire so acuity spikes don't become staffing emergencies

Physical Requirements

• Stand or walk for up to 8 hours per shift
• Support individuals with mobility needs, including transfers
• Lift and move up to 50 lbs infrequently
• Bend, kneel, reach, and climb stairs occasionally
• Implement approved physical interventions when necessary and maintain a safe environment during behavioral incidents

Compensation and Benefits

Pay: $30/hour
